    What Does This Exam Cover?

    Your examination will typically include:

    During the physical exam portion, the doctor will listen to your heart and lungs, examine your ears, nose, and throat. They will also examine your posture and spine for any abnormalities and check your reflexes and neurological functioning.

    What Do I Need To Bring To The Appointment?

    Print the New York City TLC Medical Certification Form and bring it to your appointment. Bring your NY State Driver’s License. Also, bring a list of any medications that you take, including over-the-counter pills and vitamins. Please bring your medical history and recent medical records to the appointment, as well.

    What Happens After The TLC Exam?

    The licensed medical provider will certify that they have examined you and whether or not you are medically fit to safely operate a vehicle.

    If you are medically fit to operate a TLC vehicle, you must submit the form along with your TLC application online here. You have 90 days within the date you submit your TLC application to submit the TLC medical examination form. If you do not submit it within 90 days, then your TLC application will be denied. Therefore, submit is as soon as possible.
