We are doing COVID-19 testing as per Centers for Disease Control and Prevention (CDC) and New York State Department of Health guidelines. But not everyone needs to be tested for the virus. Once you arrive at the clinic, our health care provider will help determine if you need the test.

We highly recommend that you don’t walk-in to our facilities. We have a safer process for testing and consultations. This helps in limiting the exposure of our health workers, in keeping everyone safe, and avoiding congestion in our office that would risk other staff members and patients. Due to the current situation, to protect our staff and patients and control testing capacity, we are shifting our registration process online to prioritize public health.

In order to do this, we will require a few things prior to your visit:

  1. Please check-in online and select the next available appointment. We are often booked solid. We will call text or call you if there are cancellations and a slot opens up.
  2. Confirm you have made an appointment by clicking ‘Online Check-In’ on our website or following the link sent to you. (Please remember to select the correct location and reason for visit)
    Note: If you are a physician, FDNY, or EMT and are sick, upload your ID as well. Then send us a message on our chat box and we will try to accommodate you.
  3. Once you have made the appointment, click on the yellow ‘PATIENT FORMS’ to sign our consent forms and to give us demographic information. Please attach both sides of your Picture ID and Insurance Card (if you have one) to this link. Your appointment is only final once these documents are completed and we have verified your insurance.
  4. Once we receive the forms via DocuSign, our staff will text you with a confirmation of the appointment and instructions. If our staff requires further information, we will contact you by text message or phone.
