Coronavirus Test Process at Statcare

We are doing COVID-19 testing. COVID-19 testing as per CDC and NYSDOH guidelines. Once you arrive at the location, our provider will determine that.

Don’t walk in to our facilities. Please follow the process. This helps limiting exposure of our staff and keeps everyone safe and avoids congestion in our office that would risk other staff and patients. Due to the current situation and to protect our staff and other patients, we are shifting our registration process online to prioritize health.

In order to do this we will require a few things prior to your visit. (it is recommended that you read the whole process before beginning)

  1. Please check-in online and select the next available appointment days ahead. We are booked solid. We will call text or call you if there are cancellations and something opens up. Confirm you have made an appointment by clicking online check in on our website or following this link (Please remember to select the correct location and reason for visit)
    Note: If you are a physician, FDNY, or EMT and are sick upload your ID as well. Then send us a message on our chat and we will try to accommodate you.
  2. Once you have made the appointment, click on the yellow “PATIENT FORMS” to sign our consent forms and give us some demographic information. Please attach both sides of your Picture ID and Insurance Card (if you have one) to this link. Your appointment is only final once these documents are completed and we have verified your insurance.
  3. Once we receive the forms via DocuSign, our staff will text you with a confirmation of the appointment and instructions. If there are any questions that our staff has, we will contact you by text message or phone.
